RESUMEN

The diffusion-weighted imaging signal is derived from the motion of water molecules. It represents the physiological characteristics of tissue and is an essential imaging sequence for prostate cancer. Whole-body diffusion-weighted imaging facilitates the assessment of pathological conditions throughout the body. The concept of diffusion-weighted whole-body imaging with background body signal suppression and technological advances has led to routine clinical deployment of whole-body diffusion-weighted imaging as a one-step staging tool for prostate cancer. Furthermore, whole-body diffusion-weighted imaging has the potential to reveal the state of osseous lesions, for which conventional imaging techniques are suboptimal for monitoring the response to therapy, and extraosseous lesions, and holds promise as a new imaging-based therapeutic approach. This article reviews the basics of whole-body diffusion-weighted imaging, and focuses on application of whole-body diffusion-weighted imaging in the clinical management of prostate cancer at various stages in the course of the disease.

OBJECTIVE: Dual-layer spectral detector computed tomography (DLCT) can detect noncalcified biliary stones. The diagnostic ability of DLCT for detecting biliary stones may be comparable to that of magnetic resonance cholangiopancreatography (MRCP). This study seeks to compare the diagnostic ability for biliary stones between these two imaging modalities. METHODS: This retrospective study included 102 cases with a diagnosis of biliary stones including gallstones (n = 66) and common bile duct (CBD) stones (n = 25) or spontaneously passing CBD stones (n = 11). The reference standard used was operative findings, endoscopic retrograde cholangiopancreatography or follow-up over 6 months. In DLCT, 120-kVp images, 40-keV virtual monoenergetic images and material decomposition images were created. We compared the diagnostic ability of DLCT and MRCP for biliary stones using the McNemar's test. RESULTS: The sensitivity and specificity of DLCT versus MRCP for biliary stones were 91.2% versus 95.6% and 90.9% versus 90.9%. Thus, the sensitivity and specificity were not significantly different (P = 0.25 and P = 1.0). Although in small stones (<9 mm) the sensitivity of calcified stones was not different between DLCT and MRCP (100% versus 92.5%), the sensitivity of noncalcified stones in DLCT was lower than that in MRCP (38.5% versus 100%). CONCLUSION: The diagnostic ability of biliary stones in DLCT appears comparable to that of MRCP in overall cases. However, detecting noncalcified stones less than 9 mm in size is limited in DLCT.

In this work we aimed to investigate the feasibility of using a new pulse sequence called Relaxation-Enhanced Angiography without Contrast and Triggering (REACT) for free-breathing non-contrast-enhanced MR angiography (NCE-MRA) for multiple anatomies on 3T. Two magnetization-preparation pulses were incorporated with a three-dimensional dual-echo Dixon sequence. A T2-prep pulse, followed by a non-selective inversion pulse with a short inversion time, together suppressed tissue with short T1 and T2, while enhancing the signal of native blood with long T1 and T2. A two-point non-balanced gradient-echo Dixon method, based on dual-echo acquisition with semi-flexible echo times for water-fat separation, was used for improved fat suppression over a large field of view. General image quality, vasculature visibility, and clinical indications of the proposed method were investigated in healthy subjects and patients in both torso and extremities based on visual inspection. Preliminary results from REACT obtained in free-breathing with no cardiac triggering showed uniform suppression of background tissue over the field of view and robust blood-to-tissue contrast over multiple anatomies. Future clinical studies are warranted for further investigation of its diagnostic performance and limitations.

OBJECTIVE:: To evaluate the effects of virtual monochromatic imaging (VMI) using dual-layer spectral detector CT on the image quality of coronary CT angiography (CCTA) acquired by using a low contrast material (CM) dose. METHODS:: We used a VMI 50keV protocol with a 50% CM dose (140 mgI kg-1) to scan 30 patients with renal insufficiency and a 120 kVp with the standard CM dose (280 mgI kg-1) to scan 30 controls without renal insufficiency. Quantitative parameters, including CT attenuation, image noise, and contrast-to-noise ratio (CNR), were measured. The visual image quality factors of contrast enhancement, image noise, beam-hardening artefact, vessel sharpness, and overall image quality were scored on a 4-point scale. RESULTS:: The mean CT attenuation of the ascending aorta was significantly higher for 50 keV VMI than for 120 kVp. Image noise was significantly lower under the 50 keV VMI. CNR and the mean visual score for contrast enhancement were significantly higher for 50 keV VMI. There were no significant differences in the other visual image quality parameters between the two protocols. CONCLUSION:: Dual-layer spectral detector CT using 50 keV VMI enabled reducing the CM dose by 50 % without CCAT image quality degradation in patients with renal insufficiency. ADVANCES IN KNOWLEDGE:: The VMI 50 keV protocol using dual-layer spectral detector CT and a CM dose reduced by 50 % (140 mgI kg-1) can improve the diagnostic image quality of CCTA.

Contrast-enhanced computed tomography using iodinated contrast media is useful for diagnosis of gastrointestinal diseases. However, contrast-induced nephropathy remains problematic for kidney diseases patients. Although current guidelines recommended the use of a minimal dose of contrast media necessary to obtain adequate images for diagnosis, obtaining adequate images with sufficient contrast enhancement is difficult with conventional computed tomography using reduced contrast media. Dual-layer spectral detector computed tomography enables the simultaneous acquisition of low- and high-energy data and the reconstruction of virtual monochromatic images ranging from 40 to 200 keV, retrospectively. Low-energy virtual monochromatic images can enhance the contrast of images, thereby facilitating reduced contrast media. In case 1, abdominal computed tomography angiography at 50 keV using 40% of the conventional dose of contrast media revealed the artery that was the source of diverticular bleeding in the ascending colon. In case 2, ischemia of the transverse colon was diagnosed by contrast-enhanced computed tomography and iodine-selective imaging using 40% of the conventional dose of contrast media. In case 3, advanced esophagogastric junctional cancer was staged and preoperative abdominal computed tomography angiography could be obtained with 30% of the conventional dose of contrast media. However, the texture of virtual monochromatic images may be a limitation at low energy.

We describe a case of lumbar stenosis in which retrospective spectral analysis using dual-layer spectral detector computed tomography (CT) had the ability to expand the evaluable region in the spinal canal. Spinal canal stenosis is a common condition whose symptoms (such as lower back and leg pain with walking) deteriorate the quality of life. Generally, magnetic resonance imaging (MRI) and CT myelography are performed to diagnose canal stenosis. Dual-layer spectral detector CT can yield virtual monochromatic imaging and retrospective on-demand spectral analysis without a prescan setting. Spectral analysis could expand the evaluable region in the spinal canal for increasing the contrast enhancement in the canal.

The aim of this study was to evaluate the effect of patient characteristics on the contrast agent dosage that is required to reach effective enhancement of the inferior vena cava (IVC) on computed tomography venographs (CTV).This retrospective study included 50 patients who underwent CTV at 80 kVp. The contrast injection protocol (iodine 600 mg/kg) was tailored to their body weight. We calculated the required contrast agent volume (CAVmean-IVC) to reach the mean enhancement of IVC. We performed univariate and multivariate linear regression analyses between the sex, age, body weight (BW), lean body weight (LBW), body surface area (BSA), height (HT), estimated glomerular filtration rate (eGFR), and CAVmean-IVC.The univariate linear regression analysis show that HT, BW, LBW, and BSA were significantly correlated with CAVmean-IVC (P < .01 for all). The CAVmean-IVC was significantly higher for males than females (P < .01). Multivariate regression analysis showed that BW, LBW, and BSA had a statistically significant effect on CAVmean-IVC. There was no significant correlation of age, HT, or eGFR with CAVmean-IVC.BW, LBW, and BSA each had an independent significant effect on CAVmean-IVC. The conventional BW-tailored contrast injection protocol might be insufficient for CTV.

Computed tomography (CT) is useful for diagnosing biliary stones. However, the presence of stones not detected by conventional CT, such as iso-dense stones with CT numbers similar to those of bile or small stones, is problematic. Although conventional CT provides only 120-kVp images corresponding to CT numbers at approximately 70 keV, dual-layer spectral detector CT uses one X-ray source and dual-layer detectors to collect low- and high-energy data simultaneously; retrospective spectral analysis, including virtual monochromatic images with photon energy levels of 40-200 keV, material decomposition images, and spectral curves, can be immediately performed on demand. This technique can immediately discriminate between materials with similar conventional CT numbers. Therefore, prompt and accurate diagnosis of iso-dense stones can be performed. In two out of three of our cases, iso-dense stones were detected in virtual monochromatic images at 40 keV, but in the remaining case a common 4-mm bile duct stone was not detected on 120-kVp and 40-keV images by retrospective spectral analysis. However, this stone was detected by magnetic resonance cholangiopancreatography. Retrospective spectral analysis using dual-layer spectral detector CT was useful for prompt and accurate diagnosis of iso-dense stones, but detection of <5-mm stones may be a limitation of this technique and of conventional CT.

RESUMEN

A 79-year-old woman was admitted with a left femoral neck fracture and she immediately developed circulatory shock. Echocardiography showed a markedly enlarged right ventricle (RV) with systolic ballooning of the mid-ventricular wall and preserved contractility of the apex. The left ventricular (LV) motion was normal. Multi-detector-row computed tomography showed severe congestion of the contrast media in the right atrium with no forward flow to RV, but no pulmonary embolism. She was successfully treated with percutaneous veno-arterial extracorporeal membrane oxygenation. This case presented with acute, profound, but reversible RV dysfunction triggered by acute stress in a manner similar to that seen in LV stress cardiomyopathy.

OBJECTIVE: To compare the quality of fat suppression and image quality between multiecho Dixon technique (mDixon) and spectrally adiabatic iversion recovery (SPAIR) in dynamic contrast-enhanced magnetic resonance imaging of the prostate. METHODS: This prospective study assigned thirty consecutive patients to scanning with SPAIR technique (SPAIR protocol) and another consecutive 30 patients to scanning with mDixon technique (mDixon protocol). We calculated the contrast, signal to noise ratio (SNR), contrast to noise ratio (CNR) and the coefficient of variation between the 2 protocols. Two readers compared homogeneity of fat suppression, image noise, image contrast, and image sharpness between the two protocols. RESULTS: The SNR, CNR, and contrast of mDixon protocol were significantly higher than those of the SPAIR protocol (SNR: 14.7 ± 4.1 vs 11.0 ± 2.6; P < 0.05; CNR: 6.3 ± 1.6 vs 0.5 ± 1.5; P < 0.01; contrast: 4.4 ± 1.4 vs 1.3 ± 0.5; P < 0.01), whereas the coefficient of variation of mDixon protocol was significantly lower than that of SPAIR protocol (34.7 ± 15.5 vs 43.7 ± 23.1, P < 0.01). In qualitative image analysis, the image scores for the homogeneity of fat suppression, image noise, and image sharpness were significantly higher with mDixon protocol than those with SPAIR protocol (P < 0.01). There was no significant difference in image contrast between 2 fat suppression protocols (P > 0.05). CONCLUSIONS: In dynamic contrast-enhanced magnetic resonance imaging of the prostate, mDixon technique improved the homogeneity of fat suppression without degrade of image quality compared with SPAIR technique.

PURPOSE: To develop a prediction model to distinguish between transition zone (TZ) cancers and benign prostatic hyperplasia (BPH) on multi-parametric prostate magnetic resonance imaging (mp-MRI). MATERIALS AND METHODS: This retrospective study enrolled 60 patients with either BPH or TZ cancer, who had undergone 3 T-MRI. We generated ten parameters for T2-weighted images (T2WI), diffusion-weighted images (DWI) and dynamic MRI. Using a t-test and multivariate logistic regression (LR) analysis to evaluate the parameters' accuracy, we developed LR models. We calculated the area under the receiver operating characteristic curve (ROC) of LR models by a leave-one-out cross-validation procedure, and the LR model's performance was compared with radiologists' performance with their opinion and with the Prostate Imaging Reporting and Data System (Pi-RADS v2) score. RESULTS: Multivariate LR analysis showed that only standardized T2WI signal and mean apparent diffusion coefficient (ADC) maintained their independent values (P < 0.001). The validation analysis showed that the AUC of the final LR model was comparable to that of board-certified radiologists, and superior to that of Pi-RADS scores. CONCLUSION: A standardized T2WI and mean ADC were independent factors for distinguishing between BPH and TZ cancer. The performance of the LR model was comparable to that of experienced radiologists. KEY POINTS: • It is difficult to diagnose transition zone (TZ) cancer. • We performed quantitative image analysis in multi-parametric MRI. • Standardized-T2WI and mean-ADC were independent factors for diagnosing TZ cancer. • We developed logistic-regression analysis to diagnose TZ cancer accurately. • The performance of the logistic-regression analysis was higher than PIRADSv2.

Purpose To evaluate the image quality and interobserver reproducibility of unenhanced lumbar spinal computed tomography (CT) images reconstructed with iterative model reconstruction (IMR). Materials and Methods This prospective study was approved by the local ethics committee, and written informed consent was obtained from all patients. The study included 34 patients scanned with unenhanced CT and magnetic resonance (MR) imaging for lumbar canal spinal stenosis. The CT images were reconstructed with filtered back projection (FBP), hybrid iterative reconstruction (HIR), and IMR. Image noise and contrast-to-noise ratio (CNR) were compared among the three reconstruction techniques with the repeated one-way analysis of variance. The interobserver agreement of the dural sac on all CT image sets and T2-weighted images was also compared. Qualitative analysis of the three reconstruction techniques was performed by using Friedman test and the Wilcoxon signed-rank test with Holm correction. Results The image noise of IMR was significantly lower than that of FBP or HIR (P < .001 and P < .001). Pearson correlation analysis showed that the highest correlation coefficient with interobserver agreement was with IMR (r = 0.98) followed by MR imaging (r = 0.88), FBP (r = 0.41), and HIR (r = 0.33). It also showed that the narrowest Bland-Altman limit of agreement was achieved with IMR followed by MR imaging, FBP, and HIR. The qualitative image score using IMR was significantly higher than that using FBP or HIR (P < .001 and P < .001). Conclusion IMR offers excellent noise reduction, higher interobserver reproducibility of canal stenosis, and improved image quality compared with FBP and HIR. © RSNA, 2017 Online supplemental material is available for this article.

RATIONALE AND OBJECTIVES: We compared the effect of iterative model reconstruction (IMR), filtered back projection (FBP), and hybrid iterative reconstruction (HIR) on coronary artery calcium (CAC) scoring. MATERIALS AND METHODS: CAC scans of 30 consecutive patients (18 men and 12 women, age 70.1 ± 12.2 years) were reconstructed with FBP, HIR, and IMR, and the image noise was measured on all images. Two radiologists independently measured the CAC scores using semiautomated software, and interobserver agreement was evaluated. Statistical analysis included the Spearman correlation coefficient and Bland-Altman analysis. RESULTS: The mean image noise on FBP, HIR, and IMR images was 48.0 ± 7.9, 29.6 ± 4.8, and 9.3 ± 1.3 Hounsfield units, respectively. The difference among all reconstruction combinations was significant (P < .01). The CAC score on HIR and IMR scans was 4.2% and 8.9% lower, respectively, than the CAC score on FBP images. There was no significant difference in the mean CAC score among the three reconstructions. The interobserver correlation was excellent for all three reconstructions (r2 = 0.96 FBP, 0.99 HIR, 0.99 IMR); the best Bland-Altman measure of agreement was with IMR, followed by HIR and FBP. CONCLUSION: For CAC scoring, IMR can reduce the image noise and blooming artifacts, and consequently lowers the measured CAC score. IMR can lessen measurement variability and yield stable, reproducible measurements.

RATIONALE AND OBJECTIVES: We investigated the effects of small focal spot (SFS) imaging and iterative model reconstruction (IMR) on the image quality of computed tomography angiographs (CTA) in patients with peripheral arterial disease. MATERIALS AND METHODS: We divided 60 consecutive patients with suspected or confirmed peripheral artery disease into two equal groups. One group underwent large focal spot scanning under our standard CTA protocol with hybrid iterative reconstruction (iDose(4)) (protocol 1), and the other underwent scanning with the SFS protocol and IMR (protocol 2). Quantitative image quality parameters, ie, arterial computed tomography attenuation, image noise, and the contrast-to-noise ratio, were compared and the visual image quality (depiction of each vessel) was scored on a 5-point scale. RESULTS: There was no significant difference in the arterial attenuation among all evaluated slice levels. The mean image noise was significantly lower under protocol 2 and the contrast-to-noise ratio was significantly higher at all slice levels. The visual scores assigned to the two protocols for the depiction of large vessels, such as the abdominal aorta and iliac artery, were comparable. However, the mean visual scores for small vessels in the lower extremities were significantly higher under protocol 2. CONCLUSION: CTA with SFS and IMR yielded significantly better qualitative and quantitative image quality especially for small vessels.

RESUMEN

AIMS: Patients with diabetes mellitus (DM) are likely to develop asymptomatic myocardial infarction as a complication. However, coronary artery lesions are difficult to assess in internal medicine. This study aimed to develop a prediction formula for coronary artery stenosis, as determined by coronary computed tomographic angiography (CCTA), by analyzing risk factors in patients with type 2 DM. METHODS: A prediction formula was developed based on a multivariate analysis of common factors in patients with ⩾50% coronary artery stenosis in a cohort of 327 patients with type 2 DM who underwent CCTA between 2007 and 2009, and cutoff values were calculated (derivation study). The validity of the optimal cutoff value was confirmed in a separate cohort of 317 patients with type 2 DM who underwent CCTA between 2010 and 2011 (validation study). RESULTS: In the derivation study, five predictive factors (presence/absence of hypertension, estimated glomerular filtration rate, maximum intima-media thickness [max-IMT], ankle-brachial index [ABI], and use/nonuse of diabetic medication) were used to develop a prediction formula. In the validation study, positive predictive value (PPV) and negative predictive value (NPV) of the cutoff value derived from the prediction formula were 53% and 73%, respectively. CONCLUSIONS: We developed a novel formula to predict coronary artery stenosis using five predictive factors. This formula is useful for determining whether computed tomography (CT) examination is necessary, even in clinical settings without CCTA equipment. Early detection of coronary artery stenosis in patients with DM may also lead to better health outcomes.

OBJECTIVE: To evaluate and compare the clinical utility of balanced turbo-field-echo (BTFE) magnetic resonance cholangiopancreatography (MRCP) sequences obtained at 3 and 1.5 T. METHODS: We acquired three-dimensional (3D) BTFE MRCP scans with a navigator-gated technique at 3 T on a different day after 1.5 T in 39 consecutive patients. Two radiologists independently rated the image quality and visibility of anatomical structures (right and left hepatic duct, cystic duct, gallbladder, common bile duct, and main pancreatic duct) using a four-point scale. For quantitative analysis, the signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), and acquisition time were evaluated. RESULTS: All visual scores tended to be higher for 1.5 T than 3 T images. There was a significant difference in the image quality and the depiction of the main pancreatic duct (p < 0.01). The image acquisition time was significantly shorter for 3 T than 1.5 T (199.3 ± 40.1 vs. 264.0 ± 86.5 s, p < 0.01). There was no significant difference in SNR and CNR. CONCLUSIONS: 3D-BTFE MRCP scans acquired at 3 T were of sufficient image quality with respect to the biliary tree. SNR and CNR were comparable on 3 and 1.5 T scans, although the acquisition time was significantly shorter with the 3 T scanner.

BACKGROUND: The epicardial fat volume (EFV) measured by cardiac CT has emerged as an important parameter for understanding the pathophysiology of coronary atherosclerosis. OBJECTIVE: We investigated the variability and reproducibility of EFV measurements and evaluated the effect of model-based type iterative reconstruction (M-IR) on measurement results. METHODS: Non-contrast cardiac CT data (tube voltage 120-kVp, tube current time product 32 mAs) of 30 consecutive patients were reconstructed with filtered back projection (FBP), hybrid type iterative reconstruction (H-IR), and M-IR using a slice thickness of 3.0 mm. CT attenuation and image noise was measured for all reconstructions. Two observers independently quantified EFV using semi-automated software and interobserver agreement was evaluated. RESULTS: There was no significant difference in the CT attenuation of the ascending aorta among the three reconstructions. The mean image noise on FBP-, H-IR-, and M-IR images was 48.0 ± 7.9 HU, 29.6 ± 4.8 HU, and 9.3 ± 1.3 HU, respectively; there was a significant difference among all comparison combinations for the three reconstructions (p < 0.01). FBP yielded the highest EFV among the three reconstructions (171.0 ± 54.9 cm(3) [FBP], 153.8 ± 53.1 cm(3) [H-IR], and 134.0 ± 46.4 cm(3) [M-IR]). For all three reconstructions, interobserver correlations were excellent (r = 0.91 [FBP], 0.93 [H-IR], and 0.96 [M-IR]). Interobserver comparisons showed that the lowest Bland-Altman limit of agreement was with M-IR (mean difference 2.0 ± 4.9%, 95% limit of agreement, -24.0 to 28.0%) followed by H-IR (-2.6 ± 7.1%, -39.8 to 34.6%) and FBP (-0.2 ± 8.6%, -45.3- to 45.0%). CONCLUSION: For the quantification of epicardial fat by cardiac CT, model-based iterative reconstruction can improve the image quality and lessen measurement variability.
